I got this card a couple of months ago and it's for people in the US and issued in the US: http://www.anacardusa.com/

It's a Visa Business Card with a whopping 5000 mile signup, but it's better than nothing as you can earn 1 ANA Mileage Club Mile ("AMC Mile") for every US $1 in qualifying credit card transactions

Plus:

Receive a 10% discount for ANA in-flight purchases
10% discount at all ANA group duty free shops
Special discount at IHG ANA Hotels Group Japan
10% discount at airport shops


Card issued by First National Bank of Omaha (First Bankcard) and they pull Equifax. Got a 15k limit without blushing, so if you're credit worthy, you can certainly generate some more miles on this card.
