-MCE on AA, at the time of booking
-Economy Comfort on DL, at the time of booking, and "First Class / Business Class" (or whatever they call it), at the gate.

Are the above only for the elite member, or would his travel companion be eligible as well?

well I think companion as well. I was able to select it for me and my wife MIA-ANU when we flew in October although I booked that back in January.
Edit: not 100% sure as she was also an MVP for 2014 and it was booked before the 50% charge to MVPs
