What are the busiest US airports without a lounge?
What are the largest US airports, by number of passengers, without any operating lounges (excluding USO)?

I suspect MDW would be at the top of the list and it would include a lot of Southwest dominated cities. Hopefully AmEx's new Centurion Studio concept will make it to these underserved airports.
