Many hotels today bury in the check in slip you sign upon arrival your agreement to be billed in the currency of your credit card. Read what you sign. Of course if you're checking into a hotel in Germany, you may not understand a word of the agreement you're signing. In this situation, it is very possible that when you check out, the clerk will simply say shall I leave this on your visa card (the one you presented upon check in and was pre-authorized) and you don't get a separate credit card slip showing the dcc scam being perpetrated on you until you get home. Of course, some of these shady operations will say sorry you signed an agreement to have the charge done in your currency
This has been my experience, not just at Hilton hotels either. In the very small print, usually at the bottom, there are check boxes for consent for using/selling your info for marketing and for DCC. You have to check no. OTW the default is yes for both.
