What i think i want to do is start off with a one entry tourist visa good for 60 days, then exit Thailand after 19 days, re-enter at a land border crossing and get a 15 (or 30, whatever it is, either works) day visa exempt entry, as US citizen.
It is cheaper to go ahead and get the visa than buy throw away tickets, as far as i have seen.
No, but...without a visa you may be asked by the airline for proof of onward travel.

I am not clear on your exact plans so would suggest a double-entry visa, easy-peasy.

Any 30/60 Tourist visa can be extended inside Thailand, for another 30 days.

A throw-away can be had for ~ 2,200 baht (DMK-PNH on FD). In the U.S. I think the visa fee is $40 per entry. Based on my best guess of your intentions, a single-entry followed by a exemption should suffice.
