I keep reading good things about Ayutthaya but not seeing anyway to book this with the required elephant trip. Any pointers?

Be careful putting too much reliance on TA reviews in Asia. Many companies game the system and are less than above the board.

I can't give any pointers on elephant riding as it's not something I'd ever do but let me give a nod to the recommendation for Ayuthaya. With each seasonal flood these wonderful temples are getting more and more damaged so enjoy them while you can. It's an easy train, bus, van (boat? never took that mode) ride from/to Bangkok and well worthwhile.

An afternoon beer tastes awesome on a deck overlooking the Chao Phraya after a day of touring around the island.
