In some cases, you may need to pay for fuel and you have to pay for entrance tickets to sightseeing places. The rate should be per day (10 hours). Tips are, I think, encouraged, especially when you are happy with the services (I'm not sure about this though, but I did give tips to drivers when I was in Indonesia).
