Under 7 days you call AA, book the fare, request an upgrade- it may be confirmed immediately and they then ticket the reservation. I guess in this case it isnt on 'hold' but rather is 'in ticketing' when the upgrade clears.
Agree that you should request the upgrade as soon as possible.
