Yes, why did you not check with the Gate Agent?
After you had boarded and your name fell off the UG list, the Lady's name probably became 1st on the UG List.
This lady then probably went up to the GA and asked when the 1st class upgrade was going to be done, as she was 1st on the UG List.
The GA probably told her to go and board, and that the GA would come get her, in case the UG clears.
Before we jump to a 'shena' conclusion, we should examine what could have happened. I know this has happened to me at least on 2 occasions. Having patience and waiting to board has resulted in getting the BF UG!

No they don't. And sometimes they try to do that to 'friends and relatives'.
No way the GA could have confused seat assignments, and mistake the seat of the fortunate passenger ahead of you for you.
