It's extremely simple really. To summarise: for day flights (NB: different arrangements may apply to night flights), in 90% of cases (or in some cases every time) PJs are offered, unless they are not offered in which case you should request them. Slippers are provided in the amenity pack at your seat, unless they are not in the amenity pack, in which case they will be offered to you, except for those occasions when they are not offered, in which case you should request them.

This muddle kind-of works fine if you know the ropes. It's more tough on people who aren't familiar with the vagaries of BA service. They often end up losing out like the OP because they don't know what to ask for. Incidentally it's the same confusion with socks and eye masks in CW. Maybe the seat welcome card could say what items are available on request.
