There is absolutely no reason not to seek approval for premium travel. This can be done on close to zero notice and often is and for exactly OP's reasons. He will need a letter from his doc explaining the "why".

OP needs to pursue the approval immediately and get his seat booked. The chance of an OPUP on this flight are slim to nil.
