I have flown MS a few times during this year's Ramadan - there actually IS a difference in the on-board service, at least on some routes. While it is allowed for travelers to eat while traveling, strict Muslims will often not do it anyway. As a result, MS proves boxed meals on some sectors in economy class (sandwiches etc.) as opposed to the usual (crappy) hot meals. This is, however, not the case in business class, where the meal service was much the same as usual (just fewer takers). I did not notice any change in the MS lounges in Cairo, but I honestly find the food there disgusting.
