Apple Care+ @ $99 does from day one of purchase.
When they first started +, one could only purchase apple care+ within 30 days of purchase of device. If you did not purchase at same time as device, they would actually ask you come into store to inspect.
I'm not sure how it now works - I actually called apple care and according to CS, for my 5s purchased Nov 2013, I can still buy apple care+ before end of the first year.
