Remember the chap that was presented with a hefty bill by Lufthansa for using the lounge 36 times and each time changing his flexible ticket to a new date so that he never actually flew on it?

The airline judged that to be "misconduct", and the courts upheld that view.

If an airline can provide evidence that a particular passenger has a habit of booking a certain type of ticket, which saves them a considerable amount of money based on the fare from their place of residence, or based on the fare for the segments actually consumed, and then habitually fails to complete such journeys, then the airline may very well come to the conclusion that this equates to "misconduct" of the form that allows BA under point 24.2 of the current BAEC T&Cs to empty the account of Avios.

Again, no-one is saying that missing the occasional flight is anything for BA to get worried about. But if you look at that chap that wrote to BT magazine after losing 3,000,000 miles, it would appear that ALL of his frequent transatlantic trips involved the throwing-away of the final segment. That does indeed, in my opinion, stick out and beg for action to be taken...
