Sometimes it seems based on actual miles flown, sometimes on the minimum milage I get. Here is an example:

MUENCHEN-BREMEN +250 +63 (this seems to be 25% of the 250 miles) (LG flight)
DULLES INTL-ALBANY +250 +81 (this seems to be 25% of the actutal milage) (UA flight)

Any ideas?
within Europe it's based on fixed mileage, outside Europe it's based on the actual mileage.
