The right agent can book your companion on the first flight only. Reverse works also, you fly AAA-BBB-CCC, companion gets on at BBB. Some agents (and even supervisors) don't know this is allowed but with a few calls you should be able to accomplish this and save one of the segment fees. The only hard rule is that your companion can not fly alone (you getting off while attempting to let the companion continue).
My situation is more innocuous as I am flying AAA-BBB-CCC (1 ticket, change planes at BBB), my companion is flying AAA-BBB only, so will get off at BBB. Thanks for letting me know that your situation is feasible as well.

It isn't a difficult item to book. Just call in and say you need a companion reservation and give them the flight number and date. If they give you any push back, ask for a supervisor and a CCR will do it for you.
Not sure what you mean. I can easily book the companion ticket online without any problems.
