Is there a charge when issuing a paper ticket as opposed to an E-Ricket?

I need your input, people...from USA, Canada to Europe.

Do airlines charge a fee when you issue a paper ticket as opposed to an E-Ticket?

Do travel agents charge you a fee when issuing a paper ticket? Can they issue E-tickets?

What are the travel agents charging passengers these days on top of the cost of the ticket? ie The Service Fee?

Is this fee only being implemented in North America at the moment? Are individual travellers charged as opposed to Corporate clients?

Other than the reduced commission structure that airlines across North America is doing, are Europe and Australia following suit? Has anyone heard of an airline giving out ZERO commission?

I know it sounds ridiculous here - but I'm on a fact finding mission which I need to do a comparison to what is being practiced in Asia , in particular , Singapore.
