On the booking fee, I am fairly sure you could have done that booking on ba.com? The 2-4-1 certainly appears on your BAEC account, so if you select 3 passengers in F and then apply the 2-4-1 it should make the appropriate avios reduction. If that is the case and you do something by phone which you could have done online then the offline booking fee seems entirely appropriate.
