It's not just the brakes, but the chocks as well - both would have to fail (or not be set properly) for the airplane to spontaneously roll back with a door open. Someone's in big trouble.
Didn't something similar happen a year or two ago at HKG to CX where a plane rolled and the jetbridge yanked one or more boarding doors off the plane? Seems to be a recurring problem at HKG.
