UPDATE:

The issue has been resolved and I am pleased with both United and Chase. Chase re-credited the miles to my Sapphire Preferred Account and United worked together with them to get them to do so.

The error was not mine.

Both companies have done what they should based on what happened and I have no complaint with either.
