When I applied for my Senior MC, I did it all by mail: (1)I downloaded their application form, (2)enclosed a photo + a copy of my DL, (3)I had to have the form notarized.
From info on their website, they have MTA Vans in various (scheduled) locations OR you can go to certain senior centers. I could never be at the right place at the right time though.
