I have booked hotels through the MC World Elite/Carlson program, though I haven't taken advantage of the 4th night free yet. Be advised that, like most Amex FHR bookings, the rate you will get through this program is generally equivalent to the best available FLEXIBLE rate that would be available directly from a property. So you may be able to find lower rates elsewhere that are non-refundable, promotional, or including some sort of discount (AAA, corporate, etc). You'll have to weigh what is included with the package when booking through Carlson against any lower rate you may find elsewhere.
