I witnessed it once at CCS. Pax boarded the JFK flight despite having boarding passes for MIA. What I found very surprising is that AA pax departing CCS have to undergo some of the most numerous and stringent security procedures of any AA station in the world, and yet somehow a family of four still managed to board the wrong aircraft! The problem was not discovered until they tried to take their seats and the FA thought it was a seat dupe issue. Then, the agent came on board and checked the BPs more carefully....and yikes!
