It sucks because to book good itineraries can take several calls to find a good agent, which of course means a lot of time on hold.
Maybe try a preferred challenge with US and you'll have status and a special # to call. Of course if you're an AA elite you can call the AA elite desk and ask to be transferred - that will get you to the front of the line.
The next best option is to use something like GetHuman, where a computer will call US and then ring your phone when US takes you off hold, so that at least you don't have to listen to their stupid ads while you wait.
