I searched and searched and could not find a concrete answer. I believe my search skills have weakened lately

and a bit surprised that nobody has asked this question?
Nationality: Canadian
Passport Expiry: Sometime in 2017, so nothing to worry about there
I am going to visit Japan again. I do so about once very two months on average just to kick back, relax, and bask in the culture for a few days.
On a 90-day Temporary Visitor, can I leave Japan and come back for a second visit all within the same 90-day window using the already issued 90-day Temporary Visitor sticker? Or must I get a one-time re-entry permit?
I have only done it where within the same 90-day window I visit Japan twice and all within the first Temporary Visitor 90-day sticker timeframe, but just simply got a second Temporary Visitor 90-day sticker (in other words, both stickers overlapped significantly in dates) In other words, I have never attempted re-using an already issued 90-day sticker and this time I was thinking if I could.
The former (minus the re-entry permit) is more convenient, however, if it is not allowed or if I am required to get one-time re-entry permit then I will simply do the latter again to avoid hassle.
Side question regarding the JR Pass...
(I get JR Pass Vouchers all the time and exchange them upon arrival, so no need to describe the "process")
If I happen to visit Japan twice within a three week window, can I simply get a 21-day voucher, exchange for the pass the first trip and continue to use it when I come back within that 21-day window (and leave again, this time for awhile, before the end of the 21-day window)
I don't see why this would be an issue but just curious if others have done this or know of any issues.
