Maybe the tower control is to blame? I have not listen to the records, just speculating.
The pilots are still in ultimate control, its their job to look out of the window and know whats around them regardless of what ATC says.
All I can think is the A340 was crossing diagonally, maybe the landing aircraft was just behind the window pillar and out of sight?
