I would love for someone to post the text of these surveys. Several years ago (when I was GS, and flying only UAL) I got a similar survey, but from a third party, but given where it was sent, I know it was from UAL. It asked me who I flew, what programs I kept miles in, my future travel plans, and my happiness with each program and airline. I am curious if its the same type of information, or something more targeted to quitters.
i made a post about the questions in the actual survey thread. i received the survey about 3 weeks ago and completed it. it basically asked about my travel patterns in 2013 (how many flights on UA, DL, US, AA, WN, and all OALs), then my expected patterns for 14. based on feedback others provided in that thread, it seemed to go on a dichotomy after that question, if the response was that your flying levels on UA were similar to last year it said "thanks for playing we are done here" and if not it started to ask why, with request to rank the reasons of importance as to why you choose a given airline including price, FFP/upgrades, hard product, soft product, front line customer service, etc. then there were some additional questions about how UA stacks up to other carriers in your selected areas, and some free text fields for generalized feedback.
FWIW, they promised to reward me with an RPU in 7-10 days for completing said survey. 14+ days later, nada on the RPU front. totally shocking i know.
