If the GA gave the FA a boarding pass for the passenger this would definitely be the FAs fault. OTOH, I think it's not uncommon for the GA to simply not clear people in order to get the plane out more quickly. In that case there is nothing the FA can do (and you should "blame" the GA).
You have a very valid point here. I pretty much never go back and look that hard at my class paid/flown status, so this could easily be missed, at least by me.
