- Provide a copy of your credit card statement reflecting the charges in question.
On a recent E+ refund I simply did a screen shot, cropped the United line (which contains the ticket number) and attached that .jpg to the form. It was sufficient.

I would never give them my entire credit card statement.

*edit to add: I agree it's a bunch of nonsensical busy work. I just wanted to get it done.
