Out-of-province emergency medical insurance coverage does not require using your AMEX card to pay for the trip. i.e. you are covered every time you leave your province of residence, even if you walk across the Rainbow Bridge, or go on a trip paid for by cash.
Are you referring to the non-card form of medical insurance? I would assume Crampedin13A was referring to the card-coverage version.
And if this is the case, if the incident occurred after boarding one of those revenue flights purchased with the Amex card, or before boarding the return award flight, then the basic card medical coverage should apply.
