Here's a copy of what happened from the Canadian Daily Occurance reports:
Narrative provided by the TSB of Canada Daily Notification Log 22-Aug-2003: A03F0152: C-FYKX, an Airbus A340-313 aircraft operating as Air Canada Flight 33, was departing from Honolulu's runway 08R when one of the center tires blew, and caused FOD to the runway. The flight crew did not obtain a secured gear indication after gear retraction (doors would not close). The flight returned to Honolulu and landed on runway 08L. On touch down the remaining centre tire blew and created more FOD on the runway. After landing and turning off the active runway the aircraft lost nosewheel steering. The aircraft came to rest at the intersections of 08L and 04L. The aircraft had 2 damaged center gear tires and was resting on the rims. Also the fixed gear door was detached and missing and the fairing aft of the main gear was damaged. During the takeoff the crew experienced an irregular shimmy or shift.
