If you already have a Delta FF number, you give it to Amex and they facilitate the status upgrade. If you don't have a FF number with Delta they can help get one.

Initiation fee is $5k. Cards are $2,500 per year and you are now limited to 2 supplemental cards (each at $2,500 per year). There is not a $5,000 charge for supplemental cards.
