Yes, for skiing a car is ideal, although for Hokkaido we just did plane, then bus and rented equipment. Renting a car for Hokkaido in summertime is highly recommended to see places like Shiretoko. Still wouldn't recommend the OPs north-south roadtrip though.
I don't know because I did not commute by car on expressways. I guess you have to register then.

If it had been automatic mileage and time based, then it would have applied, e.g. driving someone to NRT in the morning.
