I had the same trouble and ended up just calling yesterday and asking about two orders that haven't arrived. On one she told me it had shipped almost 3 weeks ago, and offered to reprint it for free. (I'm not sure what happens with the value of the card if the original shows up too)

On the other she told me there was a delay/hold for whatever reason, but told me she would "force it through". I'm not really sure when they didn't just do that anyway.

In cashing out Amex GC's with GCM I've repeatedly ran into a lot of problems having the perfectly correct address with no non-standard abbreviations (the customer support seems to want to spell out north rather than abbreviate it). Then I'm stuck waiting a week for the value to come back to try again. I did this 4 times on one card before eventually just using it at CVS.
