I'm no expert on this, but I do know that some of the agencies, namely Alamo, National, Thrifty, and Dollar, automatically include a second driver at no extra charge if you are in their frequent renters clubs (Insiders, Emerald Club, Blue Chip, and Fastlane, respectively). Others like Hertz and Enterprise may also offer this perk to their frequent renters club members.

Sometimes you can also get a free second driver if you rent with a particular Visa, MC, or Amex, but that varies from agency to agency according to the deals they have with the credit card companies.
