On my IST-YYZ flight, I asked if I could opt out for the main entre dishes, and could I take one fruit plate and one desert, the answer was no.
International and domestic/premium long haul meal choices are indeed different. We strive for consistency and like to offer a choice of beef, chicken, fish, pasta across all our intl/PLH flights. As Ben mentioned more catering options are being evaluated and you should see some changes in the near future.
