It can't be random. If multiple orders are going to the same address or frequent orders are placed, there must be a threshold of some kind that you reach and an order is denied. Multiple cards? Multiple orders? Amounts over a certain threshold? Who knows? I doubt web orders are scrutinized by any human. Unless someone works for Amex and can tell us otherwise, that's my theory.
