I think they're called "carry-ons" for a reason: You carry them on; that means you deal with them. AA provides a staff of people to help you if you can't manage your own bags. They are the baggage handlers.

And, BTW, I've seen many AA FAs help passengers with their carry-on bags. I don't buy this suggestion that they always refuse.
