Refund due to ill-health

I'm pretty sure I know the answer (contact your travel insurance) but on the off-chance that I'm wrong... could someone advise on the procedure if a passenger is unable to fly due to serious medical reasons diagnosed after time of booking? Flight was booked via ba.com and paid for with Amex. Traveller has some insurance but is unsure of extent of cover. Thanks.
