OTHS is a special service code we use for things like passengers with oxygen, service animals, special requests, etc. It's just a generic code.

I see a lot of Golds and MVP's with this OTHS service code and I have been told it has to do with the comp upgrade request to first class. Usually shows up if the reservation is booked through a travel agency or other airline. We usually go through and manually remove these special service codes before a flight departs to assist the crew.
