There is an incident report stored in the FA kits. And any incident happened on board, every FA that involved and purser have to write reports and turn it in to their supervisor. Due to security risk, some FA choose not to give their name to a passenger. UA policy is just giving first name by the individual. I have no issue giving my name. However, I won't give out other FA name without their consent.
Why would a UA policy of not giving out names override other standards in the case of an accident. It's more than common courtesy that both parties of any accident exchange their names and contact information. If one party refuses to give their name, in my opinion ( and I am not a lawyer

) it is like a hit and run. So if I'd be subject to a serious enough accident and the other party refuses their name, I'd call the police or if inflight insist that police would meet us on the plane. On the other side I had once a passenger dropping his laptop from the overhead bin onto my forehead. Not only did he provide his name in case something later would show, the FA who witnessed it gave me his card too. So it can be done.
