I have to say this does vary enormously, and it's a function of Club passenger numbers, staffing levels and crew aptitude.
I had 2 flights to Helsinki recently. Similar numbers of passengers, same flight, separated by a few days. In one case the purser had a hot breakfast tray in front of me before Maidstone, in the other it was Malmo. I couldn't see any difference other than staff approach / attitude. On the other hand when you have a very full CE and just one crew member doing everything, you can't expect miracles at 32,000 feet.
I've also had a LHR - HKG flight where the full CW meal tray was in front of me before Bremen, which was very impressive.
