All depends on accent, really. In most British English accents, the sounds are quite distinct (the soft 'tt' having a 't' sound, not a 'd' sound).

I would say "St Patrick's Day". To my ears, "Paddy" has slightly derogatory overtones, and I'm not sure (even being utterly non-religious) I would generally abbreviate a saint's name. I don't go to St Tommy's hospital or travel from St Panny's station.
