Check the refund status of your canceled ticket. It should tell you the make up of the refund, that should be e-Cert + cash (it is cash or CC?). That is the first proof that you should have received a new e-Cert. (New PIN).

You may also try the email route.

In the past I had the e-cert portion of refund never generated an email with the PIN, despite I received the email telling me how much the refund should be.

I ended up contact UA Insider (it was still Shannon at that time), who later confirmed that there was some system glitch at UA's end but she had confirmed with the refund processing dept that a new email would be sent. I subsequently did receive one with the PIN information.

I've learned the best option in a situation where a cancellation is necessary when using a credit like this is to call in to make the cancellation, and make sure they give you the new cert. information right then and there....they definitely can have them re-issued right away (but not sure if this is standard practice - I ask them for the information on the phone).

The new certificate information should be tied to your original itinerary. Do you still have the original e-ticket receipt they sent you, containing the ticket # and PNR? I'd call in and give them that info. (ticket # probably works best if you have it), and they should be able to see the new cert. # there. I was once actually transferred by reservations to the refund department, which I didn't know still existed on the phone, to get the info. This was probably a year or a year and a half ago. Limited hours they were open to be transferred, but got resolved after a couple of calls.

If that doesn't work, you may want to consider PM'ing the info. to UA Insider. Aaron has been quite helpful in getting FTers issues resolved when going through the normal channels hasn't worked out.
The snail mail information given is incorrect. It used to be by snail mail 3 or 4 years ago. However it has changed to email since then.

Yes, calling to cancel you would get the new PIN right on the spot.

In the past I have done all the things you mentioned on various occasions. It truly is PITA but the canceled tickets indeed have the new PIN attached and even the reservation agent could see it and gave it over the phone. My experience is also over 18 months ago..
