I mitigate this by withdrawing a lot more in Japan than I usually would do back in the States in an effort to minimise the number times I'll need to visit the cash machine and pay the fee. FWIW, I've never had any trouble withdrawing money from a cash machine in Japan and as others have suggested, always used a Japan Post Bank machine.
