Following from this, were an emergency exit door to be opened mid-flight/a window smashed out, what effect would this have? Would all the air pressure rush out and drag everything that's not securely fastened out of the plane (as I'm sure I've seen in films) with it? And would this result in some form of speedy decompression that could result in the crashing of the plane?

Again, I'm sure that these questions are highly ignorant, so please excuse this.

It would be impossible. Simply put, the door is on the inside and bigger than the hole it goes into. When flying the pressure inside is far greater than the pressure outside, pushing the door against the hole. It wouldn't fit through, and it would take a colossal force to push it inward.

If a hole was ripped in the fuselage, there would be a rapid decompression during which things may get sucked out, but it wouldn't last long. Pressure would be equalised in seconds, and you wouldn't have the entire cabin being sucked out into the abyss. The aircraft would still be under control and the pilot would fly down to a safe altitude and land.
