I can confirm that you can still buy a Serve temp card at CVS without the $2.95 fee (even though the sign on the gift card display says "thru 12/31."

The cashier (dressed like a manager-type) rang me up very quickly and didn't bat an eye at my use of a CC to load it up.

Oh, and the Green Dot MoneyPak fee is only $2.95 now, in case anyone is interested.
