I always travel with a travel pillow, unless I'm in Business Class. Also, if I have a US domestic red-eye when blankets are not provided (whether in Economy or "First"), I bring my travel blanket.

My wife swears by her cashmere travel wrap from The Travelwrap Company and is as happy as anything snuggled up in that. .
