Rather than wait it out even longer, can anyone advise on filling in the 'Request Mileage Credit' form?

'Flight Number' = just the number with EY?

'Airline Code' = The Booking class? (there';s a drop down for airline name right above)

'Ticket number' = the e-ticket number that was on my Etihad e-boarding pass?

"Flight number" will be the flight number om your boarding pass.

"Airline code" is "EY"

"Ticket number" is the ticket number - AA tickets begin with "001", EY tickets begin with "607".
