Apologies if these questions have been answered before, but I feel they are important yet hard to search for.

When AMEX says that everyday categories go from 1% to 5% cash back after $6500 spend, does that $6500 refer to those everyday categories or all purchases?

Also, will it be 5% for the rest of the calendar year (2014) or for 12 months from time of opening account (until 2/2015)?
